Reporting Forms

Private health insurers have to provide statistical and financial data on a quarterly and annual basis to PHIAC. These reports assist PHIAC in its monitoring of insurers as well as publication of industry data.  The PHIAC 1, 2, 3 and 4 templates and guidance notes can be downloaded on the Return Forms and Audit Guidance page.

In addition to the quarterly and annual reporting, private health insurers are also required to submit other forms to PHIAC such as the notification of specific events. Other ad-hoc forms relate to applications to Council to amend the status of the insurer or the health benefits fund conducted by the insurer.
